155 Beyond Risk Score: Advancing Population Health Care with Data Analytics
Risk stratification uses artificial intelligence to identify patients at highest risk and any gaps in their care. Using risk models enables resources to be directed at patients with the greatest needs and those with emerging needs. This approach leads to improvement in care outcomes and cost savings over time. This talk will review a risk stratification model and a data analysis tool. Evidence shows that gaps in care and off-site utilization have decreased.
